Sunflower’s Letter to Apply for Housing from Home

Sunflower’s Request

Thank you for the work you are doing to provide housing to people who are low income. I am happy and excited to be applying for an apartment in your lovely community.

I am writing to request a reasonable accommodation as a person with disabilities.

Reasonable accommodation request: I am requesting to complete all housing meetings and paperwork by mail or email.

Reason for request: I am mobility impaired and disabled. Due to disabling chronic illness, coming in person to your office would worsen my disability symptoms. Documentation confirming mobility impairment is enclosed.

Attached to this letter please find:

  1. A third party verification for my accommodation request
  2. A completed application for your housing program
  3. Photocopies of my state id, social security card, and birth certificate

Please process my request in accordance with FHEO HUD on Reasonable Accommodations which states that “Under Section 504, the requirement to make reasonable accommodations applies to any changes that may be necessary to provide equal opportunity to participate in any federally-assisted program or activity” and that “Permitting an applicant to submit a housing application via a different means” is a reasonable accommodation.

Sunflower’s Verification Letter

Ms. L is requesting a reasonable accommodation as a person with disabilities. She is requesting to complete all needed paperwork and meetings by phone, mail or email instead of in-person meetings. Due to her disabilities, Ms. L is mobility impaired and leaving her home to attend housing meetings can worsens her disabling symptoms. She requires this reasonable accommodation in order to participate in this program.
