Sample Disability Accommodation Letter: Meetings by Mail, Email or Phone

Dear So and So Housing Program,

I am writing to request a reasonable accommodation as a person with disabilities.

Reasonable accommodation request: I am requesting to complete all housing meetings and paperwork by mail, phone, or email instead of meetings in person in your office.

Reason for Request: I am a qualified individual with a disability, as defined by the Fair Housing Amendments Act of 1988. I have a disability which causes the following limitation: (Examples of Limitations). Due to my disabilities, I am not able to complete the application process in person.

I am requesting this accommodation so that I will have equal opportunity to participate in your housing program. HUD’s Office of Fair Housing and Equality Opportunity policy on Reasonable Accommodations and Modifications states that: “Federal nondiscrimination laws require housing providers to grant requests for reasonable accommodations and modifications in housing, programs, and activities.”

FHEO HUD policy on Reasonable Accommodations states that “Permitting an applicant to submit a housing application via a different means” is a reasonable accommodation.

Please reply in writing regarding this request for an accommodation within 10 business days. Thank you for your consideration and I look forward to receiving your reply.

Optional Extras

Optional: If you would like to request someone to come to your home: “As an alternate accommodation, if an in-person meeting is required, then I am requesting that a housing worker come to my home for the meeting.”

Optional: If they are requesting ID presented in person: “I am enclosing copies of my state id, social security card, and birth certificate.”

Optional: If you already have a copy of the application or paperwork, “I am enclosing a copy of the completed paperwork.”


Verification Letter (not needed in all cases)

Ms. L is requesting a reasonable accommodation as a person with disabilities. She is requesting to complete all needed paperwork and meetings by phone, mail or email instead of in-person meetings. (ADD reason this accommodation is needed: Examples of Limitations) Due to her disabilities, she requires this reasonable accommodation in order to participate in your housing program.
