Provider Letter: Apply from Home

This statement was included on a reasonable accommodation form signed by Sunflower’s case worker:

Ms. L is requesting a reasonable accommodation as a person with disabilities. She is requesting to complete all needed paperwork and meetings by phone, mail or email instead of in-person meetings. Due to her disabilities, Ms. L is mobility impaired and leaving her home to attend housing meetings can worsens her disabling symptoms. She requires this reasonable accommodation in order to participate in this program.
