Letter from Hibiscus’ Doctor: Exception to Porting

I am a physician and have the following credentials: MD

I have treated Hibiscus since November 2017.

Hibiscus meets the definition of a person with a disability under the fair housing act. She has physical and mental impairment that substantially limits one or more major life activities.

Hibiscus has a disability that restricts her ability to sit in a vehicle, making it difficult for her to travel long distances. She experiences an increase of muscle spasms and pain while traveling.

Hibiscus has a disability that limits her ability to leave her home and keep scheduled appointments. Her disability makes it very difficult for her to maintain emotional stability in stressful or unfamiliar situations. Her disability also causes pain and muscle spasms that restrict her ability to stand, walk, or sit for extended periods of time.

Hibiscus has established care for diagnosis/disabilities that require specialized treatment and oversight by several specialty medical practitioners. Moving to a new area, several hours away, will cause an interruption in her regular medical and mental health care.

My opinion as Hibiscus’ treating physician is that Hibiscus should be allowed to port her section 8 voucher from J County to C County. She should also be excused from any in person scheduled appointments In J County, including the appointment she missed on December 13, 2019.
