Sample Doctor Letter for Requesting a Housing Worker Come to Your Home for Meetings

Azalea made an accommodation request to have her housing intake briefing in her home. It worked! A worker from her housing authority came to her home for the meeting.

Note: We cannot guarantee that this accommodation will be approved in all cases. It may depend how far your home is from the housing office, how often you need housing meetings, and how much staff they have available. As an alternate, you can also request all meetings by phone, email or mail.


To whom it may concern:

I am Azalea Flowers primary care provider. She has been my patient since July 2019. I understand that she has requested the following accommodation for meetings with your housing office:

Having the meetings in her home due to multiple cognitive, mobility, and psychological disabilities.

(Optional: If other accommodations are needed to make meetings possible, they can be included here).

I am writing to verify that this accommodation is necessary as a result of her multiple disabilities, and to request that you make this accommodation for her.
